Client Intake To prepare for a meaningful initial meeting with

We will send you an intake information sheet to prepare for your
initial appointment. If you do not receive the intake sheet prior
to an appointment, arrive 15 minutes early to complete the intake

alternative method of divorce referred to as Collaborative Divorce,

will need to complete additional forms, some of which may include:
Inventory and Appraisement, Financial Information, and Parenting
